Mother's Day Make and Take Stained Glass class

Looking for a fun stained glass class to attend with your mother this month, our Mothers Day make and take stained glass class is the perfect girls date! This class is designed for both beginner and experienced glass artists, students will learn to grind their glass, copper foil and solder a stained glass piece. Join us for an afternoon of relaxation, creativity, music and conversation and be sure to bring your Momma!
This class is geared towards adults and teens/tweens 10+
Please note: Parker Art School does not offer refunds on missed camps or classes. However, we do offer a credit toward an upcoming class or camp when notified before scheduled camp or class.
Acknowledgment of Risks:
I, the undersigned, hereby acknowledge and understand that participating in a glass art class involves inherent risks, including but not limited to, the handling of sharp objects, tools, heated equipment, and molten glass. I am aware that these activities can result in serious injury, burns, cuts, or other physical harm.
Assumption of Risk:
I voluntarily assume all risks associated with my participation in this glass art class, including those caused by negligence or improper use of tools, equipment, or materials. I am aware of the necessary safety precautions and agree to follow all guidelines and instructions given by the instructor or other authorized personnel.
Release of Liability:
I hereby release and discharge Parker Art School, its employees, agents, and affiliates, from any and all liability, claims, demands, and causes of action that may arise out of or in connection with my participation in the glass art class, including any injury, illness, or damage to property.
This release applies to all claims of every kind, including but not limited to claims of negligence, which may result in injury, death, or property damage, whether caused by the negligence of the released parties or otherwise.
Indemnification:
I agree to indemnify and hold harmless Parker Art School and all employees and owners of from any and all claims, liabilities, losses, damages, or expenses arising out of my participation in the glass art class, including claims resulting from my own actions or negligence.
Medical Condition:
I represent that I am in good health and have no medical conditions that would prevent me from participating in the glass art class. If I am under any medical treatment, I have consulted with my physician to ensure that it is safe for me to engage in such activities. I understand that it is my responsibility to notify the instructor of any conditions that might affect my participation.
Consent to Emergency Medical Treatment:
In the event of an emergency, I consent to receive medical treatment as deemed necessary by emergency medical personnel. I understand that I am responsible for any medical costs incurred as a result of such treatment.
Photographic/Video Release:
I grant Parker Art School permission to use photographs and/or video footage taken during the class for promotional or educational purposes, without compensation or further consent. I understand that my name will not be used without my permission.
Agreement and Understanding:
I acknowledge that I have read and understand this Liability Release and Waiver Form. I am aware that by signing this form, I am voluntarily waiving certain legal rights, including the right to sue for damages. I further understand that I may contact an attorney before signing this form if I have any questions regarding its contents.
